As the COE Case Manager, you will provide coordination and connection to other community services and agencies. This role provides ongoing case management and follow-up for patients to maintain a positive patient care experience.

Requirements:
- An associate, bachelor's or master's degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services)
- Valid driver’s license in good standing
- Ability to travel up to 25% locally as needed
Responsibilities:
- Assess new referrals for appropriate level of care and SDOH/HRSN.
- Admit new patients to MMT as appropriate.
- Determines need for therapeutic medical, psycho-social, and psychiatric evaluations.
- Coordinates services with counselors, medical teammates, and external providers.
- Monitors cases by verifying patient attendance, observing, and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises and providing universal support.
- Communicating patient progress through interdisciplinary meetings as scheduled.
- Assist with discharge plans and after-care referrals.
- Assist other Pinnacle COE programs via telehealth or in person as needed.
- Provide resources to other team members.
- Participate in required annual training.
- Travel as necessary to assist with referrals, to assist and support patients in the community, and educate the community on services provided.
- Follow a teamwork philosophy when needed to help cover absent teammate duties.
- Attend team meetings and complete all training courses timely as required.
- Other duties as assigned.
