Essential Duties & Responsibilities

Reviews patient’s medical records and assigns diagnostic codes.

Maintains schedule information for clinical staff designated on the team.

Prepares correspondence for team as needed (faxes for various organizations to help facilitate the coordination of care; i.e. pharmacies, coroners, DME providers, and/or other contracting providers involved in the care).

Provides clerical support to clinical committees as assigned including agenda preparation, minutes, and notice of meetings.

Provides reports, medication lists, or other pertinent patient data to the clinical team as requested (may include but not limited to photocopying or printing documents for nursing facilities, third party payers, etc.).

Provides the clinical team with the appropriate information to facilitate care to the client.

Answers the telephone as needed for back-up support to receptionist.

Answers phone calls related to clients that are on the team and relays that information to the team.

Provides support to the clinical staff regarding medical records.
-
Provides summaries of IDT meetings to the attending physicians. Assists the team with faxing, mailing, or assimilating information regarding applications for Medicaid or other services that may be identified as needed as part of the care being provided.

Provides support to team members in the field (looks up phone numbers, addresses, or other information not available to the team while away from the office).

Enters and verifies medical coding on hospice admission charts using established regulations and guidelines.

Reviews medical coding on re-admissions to ensure codes continue to be accurate.

Documents interdisciplinary team meetings and other clinical documentation in the medical record as directed.

Enters and updates patient information in the EMR.

Gathers data and generates reports specific to the team as directed.

Sends documents, satisfaction surveys, etc. by mail or facsimile as required.

Checks documentation for completion of signatures, titles, dates and required fields.

Serves as the recorder for performance improvement groups focusing on clinical issues.

Seeks opportunities for professional growth.

Creates development goals consistent with organization and department goals.

Provides assistance with front desk coverage as assigned.
